You do not need to have everything worked out before contacting us. If you are unsure about some of the information, that is okay. Our team can talk with you about what is needed and guide you through the next steps.
What information is helpful?
When making a referral, it can be helpful to have:
- The participant’s name and contact details
- Where they live and the areas where support is required
- The type of support they are looking for
- Their current NDIS plan or relevant funding information, where available
- Their preferred days, times or frequency of support
- Their goals and what they would like support to help them achieve
- Any communication or accessibility requirements
- Information about important support needs, risks or circumstances we should understand
- Contact details for their Support Coordinator, nominee, family member or other relevant person, where appropriate.
You do not need to have all of this information before getting in touch.
Tell us what good support looks like
A referral is about more than funding and service hours.
We want to understand the person, what is important to them and what they want from their supports. This can include their interests, routines, preferences, communication style, goals and what has or hasn’t worked well in the past.
This helps us consider whether we have the right people, experience and capacity to provide support that is a good fit.
Complex support needs are welcome
The Diverse Collective specialises in supporting people with psychosocial, neurological, intellectual and complex disabilities, including people with complex behaviours and support needs.
Please do not feel that you need to simplify someone’s circumstances when making a referral. Providing relevant information helps us make an informed decision about whether we can safely and appropriately meet the person’s needs.
What happens after you make a referral?
Our team will review the information you provide and may contact you to learn more.
We will consider the person’s needs, preferences, location, requested supports and our current capacity. Where appropriate, the next step may include a conversation or meet and greet so we can learn more about the participant and they can learn more about us.
Making a referral does not mean you are committing to receive services from The Diverse Collective.
Who can make a referral?
A referral can come from the participant themselves or, with appropriate authority or consent, from someone supporting them, such as a:
- Family member or carer
- Support Coordinator
- Specialist Support Coordinator
- Plan nominee
- Hospital or health professional
- Allied health professional
- Government or community service
- Advocate or other support person.
